You'll Lose Your Mind I'm Bipolar I, and was put on Topamax after trying all the others with no luck. Had I not taken a hiatus from work I would never have been able to take this medicine for the first few months and try to handle difficult tasks. This medicine will make you stupid. You cannot remember anything, your vocabulary is reduced to that of a 6 year old, you cannot finish sentences, you cannot concentrate-it's awful. I eventually became used to this, but it really didn't "go away" for 2years. It took that long for my body to get used to it. I hear of people wanting to take the medicine to lose weight and I want to fall on the floor. I tell them you'll lose your mind and you may lose a few pounds, but they'll just come back when you get used to the medicine. Would not recommend this medicine unless you were at the end of your rope.
You have to take your time with Topamax. It's not a drug that you can expect overnight results with. SO many people push and increase it too quickly and as a result have to give up b/c of the side effects before they get to a dosage that can help their migraines. For the best results and the least problems with side effects you need to start at a low dose and SLOWLY increase the dosage. If you are still feeling the side effects at your current dose DON'T increase the dosage, wait another few days, or an extra week it will make it so much easier on you. Another thing is stay hydrated that helps so much with the tingling and dopey feeling and so does a vitamin C supplement. My severe daily migraines have decreased to about 3 a week and I'm still not at my target dose so I'm pretty happy so far. The 10lbs I've lost has been an nice bonus too.
Years on Topomax I feel topomax has helped greatly. I went from 2-3 severe migraines a month where I needed an injection of demerol/phenerghan. Now, I only get 2 severe migraines a year, needing the injection.
Worth the side effects My doc prescribed Topamax for me about 4 months ago, I have daily headaches & migraines about 2-3 times a month. I haven't had a migraine since I started taking Topamax and my daily headache is less severe. I did experience the tingling sensation in my feet almost daily for the first 6 weeks or so, but that went away. I have had some weight loss (always a plus!) and a little bit of hair thinning, but it's worth it to not have migraines anymore. UPDATE: After a couple months of hair loss I started using Rogaine for Women. I haven't lost any more hair since and I think the hair that I lost might be growing back a little bit.
Works, but uninsured beware! Yes, it works wonders on my migraines. But I don't have insurance so I either have to rely on the generosity of my expensive neurologist (that I must pay at time of visit ) to give me sample or I am up a crick without a paddle. I have had my questions to whether my hair was thinning and if Topamax was the culprit to blame but there really isn't any proof. My anxiety level has improved tremendously but I'm also on Prozac daily, which could be why I have not noticed weight loss. I have considered dropping the Prozac and raising the dosage of the Topamax. I have found no side effects from the Topamax except for a little sleepiness and that can be lessened be getting cardio exercise in daily, that is when I don't have a splitting migraine. Topamax, at my dosage, 50mg daily has not improved my hormonal monthly migraine that lasts usually 3-5 days and is severly painful.
Works well for me Topomax works well for me. I take 600mg of topomax a day, despite what the dosage chart says. When I first started I had a problem with the "pins and needles" side affect. It was pretty bad too, then my pdoc suggested taking tums with the topomax and it helped. Everytime I went up in the dosage the side effect came back until I got to the higher dosage (400 and above). It took a long time to get to a therapeutic dosage but it was worth it because I have had a hard time treating bipolar disorder and nothing else was working for me. Topomax (in conjunction with lamictal) did the trick for me. I am really suprised with how well it has worked. I do not have to take any anti depressants and my moods are mostly stable, or are not as severe as they were. Topomax is expensive and there is no generic for it yet. I would take topomax again if I had to start my drug therapy over again.

Works Great I had great results with Topamax in conjunction with Parnate. I did get headaches both times that I have used it. I had to discontinue due to arthritis in joints. This may have been the Topamax or the Parnate. We don't know. The first time that I took it I did not have this side effect, so I think that it is the combination. I would say that effectiveness wise, it evened out my moods perfectly. I have major depression and possibly bipolar disorder.
works for me Out of all the medications that I have been on, Topomax has worked the best. I am a rapid cycler and it has really helped even out my moods. The only side efect is that I have no real appetite.
Works for Me It took 3 tries to get on Topamax. I split the 25 mg dose in half and took it about twice as long and stepped up 12.5 mg at a time to 50 mg 2x a day and then in 25 mg steps to 200 mg twice a day. All at about half the rate that is recommended. I am currently taking 100 mg at bed time to control hypo manic problems, rage, hoping for some protection to brain cells should my MS become active and thankful for the 50 pounds I have lost.

My wife really likes Topamax. The under current of anger that I have fought to control all my life is gone. It makes me much easier to live with. At 100 mg it does not completely control the hypo mania but it controls it well enough that use a very little Risperdal or Zyprexa to get things under control in a hurry if I need to. It usually takes a dose pack of steroids or change in medication to trigger a hypo manic episode.

I still have a migraine headache ever few months but they are only the aura effects without pain. That beats the time when I was having the painful ones every day a 3:05 pm on the dot. But the bad ones ended long before I started taking Topamax.

I can drink Pepsi instead of Coke. Just the opposite of the pre Topamax days. But mostly I rely on coffee for my caffeine fix.
works for me I was put on Topamax after the Lithum started reaching toxic levels and was only causing my to pile on weight. I am also taking Wellbutrin and Zyprexa as well as some medications for other existing physical problems (My husband says I rattle when I walk) so sometimes it is hard to tell where the side effects are comming from. However, I have been mostly episode free for two years (except for the death of my grandmother, but that's not fair). So I guess something is working. I have my eyes examined every year by the eye doctor to stay on top of any possible problems. But the tingeling in my hands makes me nuts. The best thing to do is just be aware of any side affects and watch for them. If they pop up and are not acceptable try something else. for me, a yearly visit to the eye doc (which you should be doing anyway) and a little tigling are worth nearly two years of a peacefull family life.
Works for me I use 100mg Topamax, 200mg Lamictal and 50mg Seroquel in the morning with. Then at night 150mg WellButrin SR and 150mg Seroquel. If needed, I use 1.5 mg of Klonopin for my anxiety. This med cocktail is the best so far. I have gone trough so many since my diagnosis 2 yrs ago. I was missed diagnosed for many, many yrs. as major despressive. It wasn't until I was almost 53 that I finally was correctly diagnosed, then the fun began to find the right med combo combo. Finally, about 3 mos. ago this is what my .Dr. came up with and so far (knock on wood) it has worked. The only problem is my anxiety now. I have a problem leaving the house. I can't take the Klonopin all the time. Only when I realy need to go somewhere. Klonopin is aditictive and I have a history of being an alcoholic so I have to be extra careful. Oh, well, so be it.

Drug Uses

Seizures may be classified as either partial if they involve only a small portion of the brain or generalized if they involve more of the brain. Topamax is used in combination with other anti-seizure drugs among adults and children aged 2-16 years with partial seizures or generalized tonic-clonic seizures (in which there is prolonged contraction of the muscles of the body that causes rigidity as well as jerking motions). Topamax sprinkle capsules are approved for treatment without other drugs in patients 10 years of age and older. Topamax also is used in patients two years of age and older with seizures associated with the Lennox-Gastaut Syndrome, a severe form of epilepsy which accounts for up to 10 percent of all cases of childhood epilepsy. Children with Lennox-Gastaut Syndrome experience delays in their development and up to dozens of different, mixed types of seizures a day. The most common types of seizures in this syndrome are tonic (stiffening of the body, with the eyes rolling upwards, dilation of the pupils and shallow, irregular breathing), atonic (brief loss of muscle tone and consciousness, causing abrupt falls), myoclonic (sudden muscle jerks), and absence (staring spells).

How to use

Most commonly, topamax is started in low doses, 25 or 50 mg per day, and then increased slowly, under a doctor's orders, by 25 to 50 mg per week until an effective daily dose is reached. This slow approach to treatment reduces side effects. In children, the starting dose is up to 25 mg (1 to 3 mg/kg/day), taken nightly for the first week. The dose is then increased at 1 or 2 week intervals by 1 to 3 mg/kg/day. Although the usual adult dose is 200 mg twice daily, some adult patients may begin to see a clinical response at 200 mg per day. Some patients need doses higher than 200 mg daily. In children, the usual dose is 2.5 to 4.5 mg/kg twice daily. Patients should maintain an adequate fluid intake in order to minimize the risk of kidney stones.

Drug Class and Mechanism

Topamax is an oral drug that is used to prevent the seizures of epilepsy. (It is an anti-epileptic or anti-seizure drug). It is used primarily among patients who are not controlled by other anti-epileptic drugs. About 1 in 4 Americans diagnosed with epilepsy has seizures that resist treatment with other anti-epileptic drugs. Topamax also prevents migraine headaches. Seizures are due to the abnormal activity ("firing") of nerves in the brain, and the abnormal activity spreads to smaller or larger portions of the brain. Although topamax's exact mechanism of action is unknown, scientific studies suggest that it may alter neurotransmitters within the brain. Neurotransmitters are chemicals that nerves manufacture and use to communicate with other nearby nerves. By altering the production or action of the neurotransmitters, topamax may suppress the abnormal activity of the nerves in the brain that cause the seizures or may prevent the abnormal activity from spreading to other nerves. Other studies suggest that topamax may suppress the nerves directly (i.e., not by altering neurotransmitters) and make them less likely to fire.

Missed Dose

If you miss a dose of this medicine, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule.

Storage

Topamax tablets should be stored at room temperature, 59-86 degrees F (15-30 degrees C). Sprinkle capsules should be stored at or below 77 degrees F (25 degrees C).
Warnings/Precautions

The following medications, when taken with topamax, increase the risk of kidney stones: acetazolamide (Diamox), dorzolamide (Trusopt), methazolamide (Neptazane), dichlorphenamide (Daranide). Carbamazepine (Tegretol) and phenytoin (Dilantin) markedly decrease the amount of topamax in the body by increasing its elimination from the body. As a result, topamax may lose effectiveness unless doses are increased. Topamax may decrease the amount of estrogen in the body in women taking oral contraceptives, possibly increasing the chances of unwanted pregnancy. Patients with seizure disorders taking anticonvulsant medications, including topamax, may develop nerve toxicity from a chemical, 4'-O-methylpyridoxine, found as a contaminant in some ginkgo preparations. Do not take this medicine if you have had an allergic reaction to it or are allergic to any ingredient in this product. Do not stop using this medicine without first checking with your doctor. Some conditions may become worse when the medicine is suddenly stopped. Keep all doctor and laboratory appointments while you are using this medicine. Laboratory and/or medical tests may be done, such as measuring bicarbonate levels in your blood, before you start taking this medicine and during therapy to monitor for side effects. Before you have any medical or dental treatments, emergency care, or surgery, tell the doctor or dentist that you are taking this medicine. This medicine will add to the effects of alcohol and other depressants. Ask your doctor or pharmacist if you have questions. Do not drive, operate machinery, or do anything else that could be dangerous until you know how you react to this medicine. Using this medicine alone, with other medicines, or with alcohol may lessen your ability to drive or to perform other potentially dangerous tasks. This medicine may reduce sweating which could raise your body temperature, especially in children. The risk of this serious side effect is greater in hot weather and/or during vigorous exercise. Drink plenty of fluids and dress lightly while in hot climates, or when exercising. Check carefully for signs of decreased sweating. If this occurs, promptly seek cooler or air-conditioned shelter and/or stop exercising. Seek immediate medical attention if your body temperature is above normal, or if you have mental or mood changes, headache, or dizziness. Contact your doctor for more details. Before you begin taking any new medicine, either prescription or over-the-counter, check with your doctor or pharmacist. For women, taking this medicine while you are using birth control pills may decrease the effectiveness of your birth control pills. To prevent pregnancy, use an additional form of birth control while you are taking this medicine. Ask your doctor or pharmacist if you need more information. If you plan on becoming pregnant, discuss with your doctor the benefits and risks of using this medicine during pregnancy. It is unknown if this medicine is excreted in breast milk. If you are or will be breast-feeding while you are using this medicine, check with your doctor or pharmacist to discuss the risks to your baby.
Possible Side Effects

In adults, the most common side effects of topamax are tiredness, dizziness, coordination problems, speech problems, changes in vision or double vision, difficulty with memory, and sensory distortion. In children, the most common side effects are drowsiness, tiredness, loss of appetite, nervousness, difficulty with concentration/attention, weight decrease, aggressive reaction and difficulty with memory. Since topamax was approved, there have been 23 cases reported (as of 8-17-01) of a sudden onset of vision and eye problems. Symptoms have typically occurred within the first month of therapy, with patients reporting an acute onset of decreased vision and/or eye pain. Eye examination revealed myopia (nearsightedness), redness, decreased depth of the anterior chamber of the eye and elevated ocular pressure, with or without dilation of the pupils. Fluid accumulation within the eye may displace the lens and iris anteriorly causing secondary angle closure glaucoma. If patients develop this syndrome, the treatment is to discontinue topamax as rapidly as possible, according to the judgment of the treating physician.
